Learning Objectives
By the end of rounds you should be able to:
1. Discuss the evolving definition of SIRS/sepsis
2. Understand the components of the early and late sepsis bundle
3. Describe the approach to antibiotic use and importance of early and broad empiric coverage for patients with suspected sepsis
4. Describe the stepwise approach to vasopressor use
5. Understand measures of fluid responsiveness (including POCUS) and fluid therapies
6. Discuss adjunctive therapies for septic shock including:
corticosteroids
metabolic cocktail
nutrition
Take Home Points
Early recognition and initiation of therapies for patients with sepsis is CRITICAL to ensuring successful outcomes
Early (1-hour bundle): cultures, lactate, broad spectrum antibiotics, fluid bolus (30cc/kg)
Dosing of fluids should be tailored to patient’s volume status & underlying comorbidities
Early on consider balanced crystalloids; Later, perhaps colloids such as albumin
Source control is an ESSENTIAL and time sensitive component of sepsis therapies
Early use of vasopressors is recommended
REMEMBER: septic shock is not a hypovolemic state but rather distributive
Tailor therapies based on your patient’s (patho)physiology
For example, if they need more isotropy due to depressed myocardial function, don’t wait to “max out” on norepinephrine prior to starting epinephrine…start the epi +/- dobutamine!!
Employ dynamic measures of fluid responsiveness including passive leg raising together with use of POCUS and non-invasive devices such as the NiCom-Cheetah or FloTrac-Vigileo
Consider EARLY use of corticosteroids
